Medical Social Worker

We are looking for a passionate and enthusiastic team player to join us as a social worker who is interested in making a difference in caring for our residents and clients.

• Conduct psycho-social and financial assessments / reviews.
• Recommend and carry out appropriate clinical interventions and case management for residents and their families.
• Facilitate family conferences and attend multi-disciplinary team meetings.
• Experienced with managing the Integrated Referral Management System and the ILTC portal subvention and subsidy matters.
• Good interperesonal, organisation and communication skills.
• Competent administrative and IT skills in maintaining updated documentation of case files and time reviews of financial assistance.

Job Requirements:

• Candidate must minimally hold a Degree in Social Work.
• Registered with The Social Work Accreditation and Advisory Board (SWAAB).
